PRP050: Implementation of the Serious Illness Care Program by the Quebec COVID-19 Advance Care Planning Task Force

Abstract

Context: During the COVID-19 pandemic, many Canadians faced difficult advance care planning (ACP) decisions about life-sustaining therapies. Objective: To describe how patient-oriented research influenced the ACP policy for the province of Quebec during the COVID-19 pandemic. Study Design, Setting and Population: To help patients and clinicians in Quebec prepare for discussions about ACP during the COVID-19 pandemic, the Ministry of Health and Social Services (MSSS) in Quebec urgently created an ACP task force to generate a toolbox containing knowledge tools to support making these difficult decisions in primary care.This case study presents a narrative description of the stakeholders engaged, challenges faced, and knowledge tools created by a provincial task force commissioned by the MSSS in Québec to develop guidelines to support COVID-19 ACP in primary care. Outcomes: This study includes a description of the provincial task force, its mandate, and its methodology. Results: In March 2020, the Quebec MSSS formed its ACP Task Force with 2 patient partners, 3 clinicians, 2 ethicists, and 5 ministry policy-makers. The Task Force used a snowball technique and a rapid literature search to identify existing best-practice ACP decision support tools. Among the tools included in the toolbox, the Task Force selected the Serious Illness Care Program (SCIP), an ACP program developed by Ariadne Labs. The SCIP tools and training had been adapted and translated to French by the Patient-Centred Outcomes Research Institute-funded Meta-LARC ACP Research Team. The Meta-LARC team further engaged with the Task Force and participated in creating the provincial toolbox using the training material and tools developed for an ongoing cluster randomized trial. Making the Meta-LARC tools freely available online (https://primarycareacp.org/materials) was a facilitator in supporting the creation of Quebec’s provincial toolbox. On April 24, 2020, the provincial toolbox was sent by email to all hospital CEOs, directors of professional services and physicians in Quebec by the Deputy Minister of Health. On June 19, 2020, it was published online. Since its publication up until March 10, 2020, the tool has been downloaded 2,374 times. Conclusion: We report on the implementation of the Serious Illness Care Program by the Quebec COVID-19 ACP Task Force. This case report shows how patient-oriented research can support the development of evidence-informed policy making.
