Nomination/Submission Process
Nominator: By April 16, 2025, the nominator will submit the following materials electronically:
- Complete the nomination form - linked below
- A letter of nomination (two page maximum). The letter of nomination should emphasize why the nominee is both eligible for and deserving of the award (merit).

Please note: Once you begin your submission, you will not be able to save and return unless you click "Save and Resume." You must save the link provided, NAPCRG staff will not be able to provide you with the link.
Eligibility Criteria
The nominee must be a current NAPCRG member in good standing who has substantially impacted PaCE achievements. Examples of exceptional contributions include:
- Organized and presented PaCE preconference sessions at NAPCRG Annual Meetings.
- Organized and presented PaCE forums, workshops, or on-line webinars.
- Organized PaCE efforts to elevate the presence of PaCE both inside and outside NAPCRG
- Demonstrated impact as a Patient/Patient-Partner Board Member of the NAPCRG Board of Directors.
- Provided leadership and impact on the PaCE Committee and other NAPCRG committees.
- Led and substantially contributed to the writing of PaCE-related articles for publication.
- Demonstrated impact in Primary Care Research as an engaged team member.
Evaluation Process
Complete submissions will be evaluated independently by at least three members of the PaCE Committee. The recipient of the Patient and Clinician Engagement Distinguished Service Award will be recognized at the NAPCRG Annual Meeting.
